Like most other pump bottle adapters, the threads on the bottle aren't meant to be water-tight, just hold the bottle in place, so when the milk reaches the level of the threads, it definitely can leak out. The way to keep this from happening is to switch out or empty the bottle before the milk level rises as far as the threads.
